Billing Information
The Dental Faculty Practice is a fee-for-service practice. We offer
the following payment options:
PATIENTS WITH NO INSURANCE COVERAGE:
Payment in full is expected at the time of service. PATIENTS WITH INSURANCE COVERAGE:
We are happy to assist with filing your insurance claims, however, please understand that it is your responsibility to check your eligibility and coverage prior to your first appointment. Sometimes insurance benefits can be confusing. Please let us know if we can be of assistance. Please bring your complete insurance information with you so that we may make a copy. Non-covered services and co-pays will be due at the time of service. You will be responsible for any charges not covered by your insurance. PAYMENT METHODS
We accept Visa, MasterCard, Discover, American Express, check, money order, or cash. Please
do not send cash through the mail. If the bank returns a check,
a $20.00 return check fee will be charged to your account.
PAYMENT OPTIONS
We understand that sometimes treatment can be extended over a period of time and may require other payment arrangements. Please speak to one of our business coordinators for more information. |
